Acute and Chronic Disease Management

If your loved one has been diagnosed with an acute disease or chronic disease, our in-home care services can help reduce their likelihood of rehospitalization and help speed up their recovery time. At Thrive Together Homecare, we remain dedicated to caring for your loved one using a holistic approach that offers medication managing and administration, as well as observation and reporting of their disease while they remain in the safety and comfort of their home. Our home care services vary from companion care, to personal care, and include skilled nursing, so your loved one can receive the appropriate level of care they need while at home.

An acute disease is one that comes suddenly and has a short course, whereas a chronic disease is ongoing, and its course continues for a long period of time. Cancer Care

Cancer can be overwhelming. If your loved one has been diagnosed with cancer, we can help provide you and your loved one with the support needed during this challenging time and help reduce unnecessary distress caused by cancer. Our temporary and ongoing home care services can help ease the added burdens that cancer can bring by providing your loved one with the support they need as they experience nausea, anemia, pain, infection, and other symptoms common to cancer.

We are committed to helping your loved one remain safe and as comfortable as possible through our skilled nursing care, observation, and reporting of symptoms to our professional RN, and help with everyday activities such as personal care and light housekeeping, and companionship.

Diabetes Care

Living with diabetes can be challenging. At Thrive Together Homecare, we understand the unique care that diabetics require in order to achieve and maintain an optimum level of health. Parkinson’s Care

Parkinson’s Disease can bring about many challenges due to its progressive and uncurable nature. Oftentimes a lack of muscle control and movement caused from Parkinson’s Disease can create a concern for safety. To help reduce your loved one’s risk for falls, our professional Registered Nurse will perform an assessment of your loved one’s home environment and identify hazards upon admission. Our trained caregivers can provide hands-on and standby assistance with mobility to ensure your loved one is safe.

Our compassionate caregivers can help reduce frustration through assistance with personal care and everyday tasks such as dressing and bathing. In addition, our trained caregivers can provide healthy meal planning and meal preparation, giving you peace of mind to know that your loved one’s diet is one that supports muscle and bone strength. Our caring caregivers are also able to help encourage exercise and provide transportation to help give you peace of mind.

Rehab and Recovery Care

If your loved one is recovering from a recent hospitalization or injury, our qualified team of caregivers and nurses can help give an extra hand.
